Do I Really Need a Licensed Electrician? - IdeaMarketers.com

When you are doing home repair projects, sometimes it can be tempting just to hire a handyman or even to try to do the work yourself. After all, there are always general purpose "helpers" out there who advertise on the Internet, who put up signs or who you might hear about through word of mouth. These handymen often offer to do work at relatively low prices and you may think that this is a great deal and a good idea. In reality, however, when it comes to home improvement work, you generally get what you pay for. This means that if you want electrical work done right and done safely, you will need to hire a licensed electrician.

Why Do You Need a Licensed Electrician in Houston?

There are a number of different reasons why you need a licensed electrician in Houston, some of which are obvious and others of which may not be immediately readily apparent but which are just as important. For example, some of the reasons why you need a licensed electrician Houston include the following:

  • Only a licensed electrician can be insured and bonded.

Insured mans that the electrician is insured against any potential injury he may experience on your property (i.e. he has worker's compensation insurance) and, in many cases, that the electrician is also insured against any damage that he may cause to your home through the electrical work performed. Bonded means that the electrician has a surety bond, which is a special kind of insurance policy that protects you if the electrician doesn't do his job.

Having a licensed, insured and bonded electrician protects you from potentially being sued by an unscrupulous individual who is doing work on your home, and protects you from financial loss that might be caused by such an individual. An unlicensed person doing electrical work will be neither insured nor bonded in most cases, since he is not in compliance with the laws on licensing.

  • A licensed electrician can ensure code compliance in Houston, TX.

For certain types of installation and work performed on your home, the code requirements stipulate that a licensed electrician should perform the installation. Even when code does not require that the electrical work be performed by a licensed electrician, there are still provisions of both national and local electrical codes that must be complied with. A licensed electrician will be in the best position to ensure that you comply with all of these legal requirements.

  • A licensed electriciancan help keep your family safe

Having electrical work done by an unlicensed professional is a sure recipe for disaster. Many things can go wrong with electrical wires, ranging from shorts or overloaded circuits to serious problems that can cause a fire in your home. You do not want to take any risks with the safety of your family or the security of your property. As such, contacting only a licensed electricianis the smartest decision you can make to ensure you are making the safe choice.

Monthly bookkeeping procedures for small businesses | Spark ...

Monthly Bookkeeping ProceduresI just ran across this article at BookkeepingNews.com.? They recommend these 10 monthly procedures:

  • Reconcile bank statements
  • Reconcile Accounts Receivable
  • Reconcile Accounts Payable
  • Inventory (if applicable) should agree with end of month supporting reports
  • Reconcile loans on the Balance Sheet
  • Reconcile payroll tax payable with payroll records
  • Reconcile sales tax payable (if applicable)
  • Record depreciation and amortization
  • Expense prepaid accounts
  • Make Adjusting Journal Entries.

Whew, that?s quite a list!? And I agree that all of these steps need to be taken to keep up with the financial picture for your company.? But it really made me think,? are self-employed people really going to have time for all that?

That?s why Spark Business Consulting offers their services to the small business owner who has many other things to do with their day besides recording depreciation and amortization!? So many of my clients have told me what a relief it is to have us taking care of the monthly financial reporting,? entering of payables and receivables,? and the many other details involved in running the financial aspects of a company.

Who do you know who owns a small business???Ask them how they keep their books.? If they roll their eyes,? that might be a great referral for us.? We grow our business by referrals,? and they?are greatly appreciated.

HUNTSVILLE, TX (5/31/12) -- While the elderly represent the fastest growing segment of the population, too little is known about the nature and scope of crime impacting this generation. Victoria Titterington of Sam Houston State University is trying to change that with a series of studies that examine the elderly and crime.

The elderly, defined as persons over the age of 65, currently represent about 12 percent of the population nationally. But with the baby boomer generation aging, that number is expected to double by 2030. While the elderly are only a minority of crime victims, they are at higher risk for property crimes, self-neglect and abuse, Titterington found in a 2010 report she prepared for the Texas Crime Victims' Institute.

Reports of elder abuse in domestic settings have increased dramatically, according to several sources, and the University of Chicago's National Social Life, Health & Aging Project estimated that 13 percent of older Americans suffer some form of abuse. Titterington said those figures may be grossly underestimated; for example, the 2004 Survey of State Protective Services said elderly abuse victims range from 100,000 to one million annually. Further, female seniors are at greater risk for abuse than older men.

Financial swindles are one of the fastest growing forms of abuse, according to the National Center for Elder Abuse in their 2009 statistics. The elderly also suffer more self-neglect, and these particular victims are generally depressed, confused or extremely frail.

In one study, Titterington and Napoleon Reyes of Sonoma State University studied homicide and the elderly in three major cities, including Houston, Chicago and Miami, between the mid-1980s and the mid-1990s. This research indicates that compared to younger victims, older homicide victims are significantly more likely to be female, to be killed by family members, and to be killed in the course of a robbery or other felony.

In another study, co-authored with SHSU Ph.D. student Jeanne Subjack, the two examined the overall and spousal sex ratios of homicide offending in Texas' six largest cities. The results showed that these sex ratios are low for homicide in general, meaning that women represent a significant minority of offenders. For example, for the period of 1976-2007, for every 100 male offenders in El Paso, there were only 10 female homicide offenders. However, women's representation as homicide offenders is significantly larger in the case of spousal homicides. Across these six cities for this 30-year period, there was a range of 17 to 96 female (and a mean of 56) spousal homicide offenders for every 100 males who were killing spouses/intimate partners. Though the spousal sex ratios of killing were large for both younger and older couples, the levels of spousal homicide offending for males and females has seen a significant reduction in these cities, as well as nationally.

Historically, the criminal justice system has not been a major player in elder abuse, but more and more police are dealing with these issues on the street. In fact, federal legislation, The Elder Justice Act, was passed in 2010, as part of the Patient Protection and Affordable Care Act. It is designed to address many of the weaknesses in current federal and state programs dealing with the abuse, neglect and exploitation of older people.

Younger colon cancer patients have worse prognosis at diagnosis ...

Younger patients with colorectal cancer were more likely to present advanced stage tumors at diagnosis and metastasize much sooner, yet had better than or equal survival to patients 50 and older, according to data being presented at the 2012 American Society of Clinical Oncology Annual Meeting in Chicago. (Abstract # 3621).

The study was led by Edith Mitchell, M.D., a clinical professor in the Department of Medical Oncology at Jefferson Medical College of Thomas Jefferson University. Dr. Mitchell is also Director of the newly-established Center to Eliminate Cancer Disparities at Jefferson's Kimmel Cancer Center.

"We're seeing more advanced tumors in this population because the cases aren't being caught early enough," said Dr. Mitchell. "Screening isn't recommended until age 50, and the younger a patient is, the more likely they are to ignore symptoms of more advanced stages of the disease."

The objective of this study was to assess pathological features and outcomes of colorectal cancer in patients less than age 50 using an institutional sample and comparing to the Surveillance, Epidemiology and End Results (SEER) database.

Dr. Mitchell and colleagues obtained data from the tumor registry of Thomas Jefferson University Hospital (TJUH) on 4,595 patients treated for colorectal cancer from 1988 to 2007. They compared those data with data obtained from the National Cancer Institute's Surveillance, Epidemiology and End Results (SEER) database on 290,338 patients with colorectal cancer treated from 1988 to 2004. The researchers collected data on location, stage and histologic grade of the cancer.

Patients under age 50 with colorectal cancer presented with more advanced stage tumors in both data sets (SEER and TJUH), and had more poorly differentiated tumors than older patients, the researchers found. Patients under 50 also had more mucinous/signet ring cell tumors with 12 percent to 8.1 percent in the TJUH data and 13.2 percent to 10.3 percent in the SEER data, with younger males having the highest prevalence in both data sets.

Younger patients had fewer right-side tumors than patients 50 and over, and a higher proportion of rectal tumors. Patients under age 50 were also more likely to have positive nodes at all stages relative to 50 and over, as well as more likely to develop peritoneal metastases, but less likely to have lung metastases than older patients.

Despite their poor pathologic features, patients under age 50 had better than or equal survival to those 50 and older, which may in part be explained by their overall health. Early evidence suggests that younger patients are able to tolerate more aggressive cancer therapies because of fewer co-morbidities, said co-author Scott Goldstein, M.D., of Jefferson's Department of Surgery.

"Ongoing studies will help clarify the survival disparity and assess differences in treatment and molecular features between younger and older colorectal cancer patients," Dr. Mitchell said.

Fish farm in 14th-floor Bronx apartment?

NEW YORK -- A Bronx man who runs a fish farm in his 14th-floor apartment is being sued by his landlord in a building where neighbors claim the tanks leak and stink up their homes.

The landlord claims Christopher Toole, 47, is violating his lease at the Riverdale building by illegally breeding fish and running the Society of Aquaponic Values and Education from the home, according to the New York Post.

He has "refused to refrain" from making noises and causing odors, the lawsuit says.

Read the story at NBCNewYork.com

"He's running a business out of his apartment," Errol Brett, the landlord for Windsor Apts. Inc., told the Post.

Neighbors say Toole is responsible for several large leaks and drags his fish farming materials across the floor through the night.

?It?s irritating because you hear noise all the time. It?s 3:30 in the morning, and you hear him dragging his aquarium or whatever it is across the floor. It has changed my life,?? resident Roch McDowell, who lives directly below Toole, told the paper.

Toole formerly worked at Morgan Stanley and served as a vice president for Sovereign Bank, according to the Post. He now teaches kids about fish farming and sells his tilapia.

He told the Post he was not aware of the suit, but said "any publicity is good publicity."

President Obama honors veterans during Memorial Day weekend

WASHINGTON ? President Barack Obama is paying tribute to veterans during Memorial Day weekend, honoring those willing to sacrifice their lives for their country.

Obama says in his weekly radio and Internet address he will mark Monday?s holiday with veterans and their families at Arlington National Cemetery and the Vietnam Veterans Memorial.

The president says the nation needs to honor veterans, including those who served in Iraq and Afghanistan, by ensuring they have access to health care, higher education and jobs.

In the Republican address, congresswoman Lynn Jenkins of Kansas says higher prices from gasoline to groceries have placed a strain on families. She is calling for building the controversial Canada-to-Texas Keystone XL oil pipeline.

The Investigation on the Retention of Cervix Uteri and ...

[Abstract]

Objective: To investigate the value of retention of cervix uteri and essentiality ofappendectomy in the epithelial ovarian cancer (EOC). Methods: The clinical data of 145patients were analyzed retrospectively, which were in the Guizhou Provincical People?s Hospital and the Affiliated hospital of Zunyi Medical College from 1999 to 2008. The metastatic circumstance about clinical pathological data in omentum, appendix and cervix uteri were specifically analyzed according to international federation of obstetrics and gynecology (FIGO) stage, pathological type, pathological grade. The expression of VEGF-C and MMP-9 were detected by Streptavidin Peroxidase (SP) method in 30 cases of EOC and 20 benign ovarian tumor and they were analyzed in omentum, appendix and cervix uteri, which were in the Affiliated hospital of Zunyi Medical College from 2003 to 2008. Results:1. The analysis of clinical pathological data: There were 63 cases omentum metastases in 118 cases omentumectomy and it?s metastases rate was 53.39%. The metastases rate of omentum in serous cystadenocarcinoma was higher than that in mucous cystoadenocarcinoma (P < 0.05) .The metastases rate of omentum in histological grade step up from well differentiated to poorly differentiated gradually (P<0.05). There were 16 cases appendix metastases in 57 cases appendectomy and it?s metastases rate was 28.07%. There were no appendix metastases in 19 cases early EOC patients. There were 16 cases appendix metastases in 38 cases advanced EOC patients and it?s metastases rates were 42.11%. The metastatic circumstance of EOC in appendix was closely correlated with FIGO stage. But it was not correlated with histological type and histological grade (P>0.05). There were no cervix uteri metastases cases in 134 cases total abdominal hysterectomy. 2. The positive expression of VEGF-C and MMP-9 protein were found in tumor cell intracytoplasm. The positive expression of MMP-9 protein was also found in some cellula intersitialis. The expression of VEGF-C and MMP-9 in the tissues of EOC were statistically different from those in benign ovarian tumor (P< 0.05). But they were not correlated with the FIGO stage, histological type, and histological grade (P>0.05). 3. The expression of VEGF-C and MMP-9 in metastases of omentum and appendix were not statistically different from those in the tissues of EOC (P>0.05) .The expression of VEGF-C and MMP-9 in normal mentum, appendix and cervix uteri were statistically different from those in the tissues of EOC(P<0.05). 4. The expression ofMMP-9 and VEGF-C were not correlated with each other. Conclusoins: 1. The EOC wasnot easily metastasize to cervix uteri, so it is reasonable alternative to retain cervix uteri in young patients, early stage of EOC and difficult surgery in advanced stage. 2. The metastatic circumstance of EOC in appendix was closely correlated with FIGO stage. But it was not correlated with histological type and histological grade. According to goals of treatment and reducing perioperative complications, we claimed the appendix may be retained in early stage and be properly removed in advanced stage. 3. VEGF-C and MMP-9 were highly expressed in primary and metastatic ovarian carcinoma, and might play an important role in the onset and progression of ovarian malignant tumor. VEGF-C and MMP-9 were less expressed in cervix uteri, that presumed EOC was not easily metastasize to cervix uteri, then it is reasonable alternative to retain cervix uteri. 4. The over expression of VEGF-C and MMP-9 might be the molecular marker for the infestation and metastasis of epithelial ovarian carcinoma.

Super-sensitive tests could detect diseases earlier

ScienceDaily (May 27, 2012) ? Scientists have developed an ultra-sensitive test that should enable them to detect signs of a disease in its earliest stages, in research published May 27in the journal Nature Materials.

The scientists, from Imperial College London and the University of Vigo, have created a test to detect particular molecules that indicate the presence of disease, even when these are in very low concentrations. There are already tests available for some diseases that look for such biomarkers using biological sensors or 'biosensors'. However, existing biosensors become less sensitive and predictable at detecting biomarkers when they are in very low concentrations, as occurs when a disease is in its early stages.

In the new study, the researchers demonstrated that the new biosensor test can find a biomarker associated with prostate cancer, called Prostate Specific Antigen (PSA). However, the team say that the biosensor can be easily reconfigured to test for other diseases or viruses where the related biomarker is known.

Professor Molly Stevens, senior author of the study from the Departments of Materials and Bioengineering at Imperial College London, said: "It is vital to detect diseases at an early stage if we want people to have the best possible outcomes -- diseases are usually easier to treat at this stage, and early diagnosis can give us the chance to halt a disease before symptoms worsen. However, for many diseases, using current technology to look for early signs of disease can be like finding the proverbial needle in a haystack. Our new test can actually find that needle. We only looked at the biomarker for one disease in this study, but we're confident that the test can be adapted to identify many other diseases at an early stage."

The team demonstrated the effectiveness of their biosensor by testing PSA biomarker samples in solutions containing a complex mixture of blood derived serum proteins. Monitoring the levels of PSA at ultralow concentrations can be crucial in the early diagnosis of the reoccurrence of prostate cancer, but classic detection approaches are not sensitive enough to carry out this analysis with a high degree of accuracy. The new test could enable more reliable diagnosis, but more research will need to be done to further explore its potential.

In their study, the team detected PSA at 0.000000000000000001 grams per millilitre, which is at the limits of current biosensor performance. By comparison, an existing test called an Enzyme-Linked Immunosorbent Assay (ELISA) test can detect PSA at 0.000000001 grams per millilitre, which is nine orders of magnitude more concentrated.

The biosensors used in the new study consist of nanoscopic-sized gold stars floating in a solution containing other blood derived proteins. Attached to the surface of these gold stars are antibodies, which latch onto PSA when they detect it in a sample. A secondary antibody, which has an enzyme called glucose oxidase attached to it, recognises the PSA and creates a distinctive silver crystal coating on the gold stars, which is more apparent when the PSA biomarkers are in low concentrations. This silver coating acts like a signal that PSA is present, and it can be easily detected by scientists using optical microscopes.

The next stage of the research will see the team carrying out further clinical testing to assess the efficacy of the biosensor in detecting a range of different biomarkers associated with conditions such as HIV and other infections. They will also explore ways of commercialising their product.

This research was funded by the European Research Council and via a Marie Curie fellowship.

Fixing Up Wire Deck Railing Without The Complications | Patio Deck ...

Wire Deck Railing

Current trends in home improvement show that an increasing number of house owners opt to have their houses renovated for long-term residence than move to a new property. The current state of real estate has influenced the changed mindset of home owners nowadays. Investments are being made to homes raising its landholdings value, not to forget, injecting their personal style. In the past, many home owners have concentrated on having their interiors made better, however this year, residents are investing more in outdoor improvements from the top to bottom.

For simple yet elegant homes, the residents may choose composite deck railings and those who live in modern style houses may choose wire deck railings. There are various designs available for deck rails that are suitable for any architectural type. For do-it-yourself fans, there are simple ways of installing deck railing without the assistance of professionals.

Many agree that cable deck railings are easily installed compared to its wooden board counterparts. The helpful deck railing installation tips are as follows:

* Purchase wire nettings at the hardware store or animal supply shop. The mesh is commonly used in industrial jobs or farms with poultry to enclose chicken coops for instance. There are a large number of basic wire designs that can improve the aesthetics of your outdoor area and at the same time securing your deck.

* Cut the wire pieces into parts. If the distance between the rail posts are four feet (the ideal distance), the parts should be about the same length. Firmly fix the cable in various areas to check if the wire is stretched tightly and stay in place independently. Staples or hooks will be fine to use for attaching the netting.

* Further secure the wire firmly. Add support by placing an extra slab of wood over the connection spots to increase wire firmness and at the same time obscure the cable joints. Hammer the added parts into place; applying paint would make this part look better. Some fasten the wire behind the railing to avoid eyesores.

In the home improvement industry, there are numerous companies providing materials for deck installation nowadays, assuring the quality and durability of your decks which makes the buyers choose from a wide range of options. No matter what type of deck design householders decide on choosing will be a possibility because of the industrial developments.

An easy to install wire deck railing needs maintenance, good thing, this type just requires basic cleaning. Decks are an expansion of a home?s indoor areas and are great places to do activities that are private or involving the community. What better place to make these memories than on the deck of your dreams.

Keeping Company Lines Available With An Advanced Telephone ...

?

If you?ve been through making a phone call for an enquiry during the ?80s, maybe you wasted hours upon hours of your life waiting for a customer service representative to attend to you. People who had to ask or report concerns would often physically head directly to the company establishment itself. That basically illustrates the inadequacy of the business telephone system decades in the past. Currently, however, businesses place importance about maintaining their company communications available that they actually hire workers to work on all phone-in inquiries. Several even outsource this arm of their business due to the fact it?s far too large task for their regular staff to deal with.

Businesses these days stick to technological help to enhance their sales. It is important that they?re in line with the most current equipment technology can offer.

Technologies have made all the difference for customer care; a telephone system already has so many functions that even if the operators or customer service staff tend to be flooded with calls, going through the hotline is no longer an issue. Apart from this, calls can also be possible without an actual phone! Many utilize their computers today to be able to call long-distance and companies are taking complete benefit from the Voice over Internet Protocols (VoIP) system to be able to update and totally develop their customer or client help services effective. VoIP less expensive too and the rates remain the same no matter where the call originated from.

The innovative technology of Voice over internet protocol could deal with multiple-calling also, that enables one person to call a group of people for one dial. It isn?t really the normal sort of call which customers make for questions, but this is utilized for seamless meeting calls that many international companies make to keep an eye on their own businesses? activities all over the globe. Since many organizations have been required to outsource as a consequence of the global financial crises, this type of communication system has become one of the more core necessities preserving businesses completely operational.

Also analogue telephone systems have experienced an incredible upgrade over the past decade and they have more call features that ultimately ensure interaction faster and simpler. This then, enables small businesses to accommodate their clients? demands better, even with the simplicity of the phone system. Lots of analogue phones now have the call-back feature that enables returning missed phone calls a piece of cake; this kind of function is also responsible for minimizing the rate of prank calls over the years simply because tracking the call is definitely as simple as pushing a button.

Technology has created all the difference regarding customer support; a telephone system presently provides a lot of features which makes getting through a hotline no longer an issue. Furthermore, calls can even be made without an actual telephone. Check out Media Updates for more information.

Brunetti ? myloves(:

mm yummy i?ve been going around trying all these lovely brunch/hightea places recently. it?s just sooo fun looking at desserts! :):) can drool over them anytime <3

interestingly, brunetti was introduced to me by one of my profs. it?s pretty famous in melbourne and it came to singapore just last year! have been dying to try it since then, can?t believe i actually waited half a year to finally get here! haha. it?s described as a cafe, pasticceria (pastry shop), gelateria (i?m sure you can guess this ? gelato store!), paninoteca (sandwich bar), panetteria (bakery) and cioccolateria (chocolate shop) all in one. OMG I WANNA GO TO ITALY FOR EXCHANGE ALREADY!

ah, just look at the rows and rows of desserts! it?s absolutely?irresistible?:D brunett is tucked away in tanglin mall, and i think this could possibly be the first time i?ve ever stepped in here! :O haha i mean, there isnt really much reason for me to come here, it?s not very accessible :/ but i realised there are quite a lot of hidden finds here! there were at least two other cute looking cafes here other than brunetti, which i would love to try out.

anyways, on to the food :D

we came for their brunch, although probably high tea would have been better! their brunch menu was fairly limited in my opinion (considering how we were spoilt for choice at antoinette) but still, it was not bad! :) this time i ordered by favourite Silician Eggs Benedict ($12.90). after skipping my eggs the last time, i couldnt give it up again, could i? ;) it was so delicious! i loved how the luscious hollandaise sauce wrapped the wobbly poached egg which exploded with the runny yolk when i pried it open. it flowed all over the crispy baguette with italian ham and cheese. mmmm :)

something unconventional i havent seen on other brunch menus would be the Italian Farmers Breakfast ($11.90) which was an interesting creamy polenta (a type of grain) with scrambled egg, sausage and truffles. it was a homely dish, but i felt that it could have been a little tastier. i couldnt really detect the flavour of the truffles :( but maybe it was because i took a really small mouthful.

papa ordered the wild mushroom omelette ($11.90) that came studded with mixed herbs, mushrooms and cheese, served with rustic toast and sausage. the egg was wonderfully creamy and i loved how the cheese added another dimension of flavour to it! the sausage was nice and springy too.

the italian pan eggs ($14.90) was such a cute dish! haha dont you think it looks adorable? ;) sunny side up eggs with bacon and tomatoes cooked in a cast iron skillet and garnished with fresh basil and black truffles xD yummy! the taste of the herbs was brought out delightfully, and we loved mopping up the egg with the garlic breadsticks that came on the side.

last breakfast dish were the roma buttermilk pancakes ($9.90). topped with bananas and strawberries and drizzled with honey ? comfort food!!

i?m so silly. to think that i was so excited about trying brunetti?s italian hot chocolate! but i didnt realise they had two types here!!! :( so instead i ordered the regular hot chocolate ($6.50) sigh sigh sigh :( such a waste! i really heard raving reviews about the italian hot chocolate, and brunetti is probably the only place in singapore where i could find it! well, this hot chocolate was alright, quite thick, but it wasnt the one i was looking for!!!

we were so full already after all our food and drinks, but the entire counter of desserts was beckoning to us and we absolutely could not leave without trying something! we spent the longest time oogling at the desserts trying to decide what to order. everything just looked so good.

first up, the Bacio ($8) was a dark chocolate and hazelnut bavaroise that looked absolutely gorgeous!!! dont you think it looks so simply classic? :) but i think because it looked so good, i felt that the taste didnt meet my expectations? it was interesting, but not really what i expected. there was a combination of textures in the slightly thick chewy exterior which encased a soft chocolatey mousse within.

i loved the simplicity of the new york cheesecake ($10) ? can?t go wrong with this! :)

and finally, italian gelato <33333 if i really go to italy for exchange, i am so gonna get fat on this :D it was sooo lovely. just look at my first photo for this post. it looked so delicious! :D :D :D because we wanted to try more than one flavour, we got the medium cup for $7.50 and got belgium chocolate and cookies :) it was smooth, creamy and so deliciously melt-in-your-mouth lovely :) if you can?t decide on a dessert to get, skip the cakes and get the gelato. it was THE BEST.

XOXO

Kelly

New program seeks transplant candidates for 5-year clinical trial

The UCLA Health System has launched the UCLA Face Transplantation Program, the first surgical program of its kind in the western United States and one of only a handful in the nation.

"Facial transplantation offers the potential to restore humanity to persons who have suffered the devastating loss of their face," said Dr. Kodi Azari, chief of reconstructive transplantation and associate professor of surgery at the David Geffen School of Medicine at UCLA. "People with massive facial injuries often have trouble breathing, speaking and eating, as well as depression and social isolation. Early surgeries have demonstrated very promising results in improving both appearance and function."

The UCLA face transplantation team recognizes the sensitivity required when it comes to working with patients who have experienced a severe facial disfigurement. Understanding that a person's identity and sense of self are closely tied to their facial appearance, the team will also support the patient's emotional adjustment to their new face after the surgery.

"Our goal in creating this program is to return a sense of normalcy to our patients' lives," said Dr. Reza Jarrahy, surgical co-director of the new program and assistant professor of plastic and reconstructive surgery at the Geffen School of Medicine. "We hope that restoring facial form and function will provide the opportunity for patients to lead productive lives that are not defined or hampered by facial appearance."

UCLA is currently seeking patients willing to participate in a face-transplant clinical trial and to be followed for five years after their surgery.

Candidates for the clinical trial will undergo a thorough evaluation to determine whether they meet the criteria for participation. The evaluation includes a comprehensive medical history, a physical examination, lab tests, X-rays and a psychological exam. Approved participants will be placed on a waiting list until the center identifies a suitable match from a donor. Recipients must match the donor's blood type, gender, ethnicity, skin tone, hair pattern and other criteria.

Additional eligibility criteria for the clinical trial include:

  • The patient's facial disfigurement cannot be repaired by conventional surgery.
  • The disfigurement is not due to a birth defect.
  • The patient's age is between 18 and 60 years.
  • The patient has no serious infections, including HIV or hepatitis B or C.
  • The patient is in otherwise good general health.
  • The patient must commit to extensive rehabilitation after surgery, including soft-tissue massage and speech, swallowing and facial-movement therapies.
  • The patient must agree to follow a drug schedule to prevent transplant rejection and attend all appointments at the transplant center.

Dr. Gerald Lipshutz, medical director of the new face transplantation program and associate professor of surgery and medicine at the Geffen School of Medicine, noted that face transplantation is still considered experimental and is not without risk.

"Each patient will need to take drugs the rest of their life to suppress their immune system and prevent rejection of their new face," Lipshutz said. "One of our study's purposes is to look at the effectiveness and safety of the anti-rejection drugs that will be used."

To date, 19 patients worldwide have received partial or complete facial transplants; five of these surgeries have been performed in the United States.

The surgery takes from eight to 20 hours to complete. Surgeons first remove the damaged portions of the patient's face and then attach the donated face to the patient's supporting structures. This includes joining soft tissue like skin, fat, muscles, tendons and ligaments and securing the bones with screws and other hardware. The surgery's most delicate facet involves painstakingly stitching the patient's nerves and blood vessels too small to be seen by the naked eye to those in the new face.

While solid-organ transplants are common at UCLA and other major medical centers, reconstructive transplantation a complex surgery involving a variety of tissues (including bones, tendons, arteries and nerves) marks a new direction for the field. Unlike organ transplants, which are performed to save lives, reconstructive transplants aim to dramatically improve them.

"Microvascular transfer of tissues to reconstruct the face is not new. It is very similar to reconstructive surgery after traumatic injury," said Dr. Ronald W. Busuttil, distinguished professor and executive chairman of surgery at the Geffen School of Medicine, who established the UCLA Liver Transplant Program in 1984, the first on the West Coast. "What makes this study experimental is that we are uniting the fields of microvascular reconstructive surgery and transplantation medicine to transplant the face."

The demand for face transplantation procedures is expected to increase due to long-term U.S. military action overseas. Experts estimate that some 200 veterans have lost all or part of their face. In the civilian sector, nearly 1,000 trauma and burn patients suffer extensive facial injuries that drastically affect their lives.

The UCLA Face Transplantation Program plans to partner with UCLA's highly successful Operation Mend, which offers facial and hand reconstructive surgery to the nation's wounded soldiers.

The program will integrate specialists from throughout the UCLA Health System, including the areas of head and neck surgery, plastic and reconstructive surgery, oral surgery, psychiatry, pathology, anesthesia, internal medicine, radiology, neurology, ethics, and rehabilitation services.
